Our clinician-led medical practice consists of more than thirty physicians and Advanced Practice Providers (APPs). With a focus on both patient well-being and provider satisfaction, our practice model is designed to deliver outstanding care while fostering flexibility and fulfillment for our providers.

Meet Our Founder
Dr. Peter Jian, M.D., FAAFP, is the founder of Journey Medical Care Group, with a focus on post-acute and long-term care. He earned his Doctor of Medicine (M.D.) degree and completed his family medicine residency at Baylor College of Medicine in Houston, Texas. Early in his career, Dr. Jian served as an academic physician and was honored with the highest early-career award at Baylor College of Medicine.
Driven by a passion for serving elderly, debilitated, and disabled patients, Dr. Jian formed Journey Medical Care Group, where he oversees clinical operations and spearheads the practice's growth. Additionally, as medical director for several skilled nursing facilities, he focuses on driving clinical excellence and improving health outcomes for every resident.
Dr. Jian believes that each life is a journey to be nurtured and cherished at every stage. He strives to treat patients the same way he treats his own family. Dr. Jian is married and have a toddler boy. In his spare time, he enjoys hosting small group fellowship meetings, traveling, and salsa dancing.
